Registration and General Information Forms

All competitors must complete ONE Medical Information and Waiver Form available below in pdf format. Also, each competitor must complete an entry form for each competition entered, also available below in pdf format. For example, if entering as part of a team, your name should be on the team entry form, and if you also enter an individual division, an Individual Entry Form should be submitted also.

Return the form(s) with the appropriate entry fee(s) to the address below.

Entry forms can also be turned in to both Surf City and Aussie Island surf shops up until the weekend of the contest (Friday, October 3rd). No late fees will be applied! :-) Besides a great day on the beach with your family and freinds, your entry fee gets you a contest t-shirt, a catered dinner from K38, and a donation to the Cape Fear Chapter of Surfrider Foundation so we can continue to good things in our community.

Registration for the TEAM EVENTS is due September 26, 2008

SURF SHOP TEAM CHALLENGE

(20 teams) $125/per team

Please Note:

• Each team must have five members.
• Teams entered into the SURF SHOP TEAM CHALLENGE must have one participant in each of the categories.

Team Categories are as follows:

• Grom: UNDER 18
• Female: Any Age
• Adult: 18-31
• Master: 31+
• Long Board: Any Age

College Surf Tag-Team Event

(10-12 teams, high school teams accepted on a space available basis) $50/per team
College teams must have at least one female. Also, a faculty member on your team is encouraged. Faculty members will score double points on any one wave during competition.

OPEN DIVISIONS

Registration for the OPEN EVENTS will remain open until the event is full.
